The Key Services Case Management is a team of experienced
Case Managers and Social Workers with extensive experience in assisting families, veterans, disabled adults and seniors navigate life planning and their overall care.
We understand caring for a loved one and/or an aging parent can be overwhelming at times especially with not knowing what to do, where to go and what resources are available to assist them.
Our Case Managers and Social Workers are advocates for your loved one, an extra set of ears and eyes when you are not able to do so; while assisting in making informed decisions that add to their quality of life.

We conduct a personal assessment to determine your overall needs medically, socially and environmentally.
CONNECT
We will connect you with the resources and people needed to add to your quality of life and daily living.
ADVOCATE
We advocate on behalf and in the best interest of you and your family. We are an extra pair of ears, eyes and at times a mouthpiece to make sure you receive the quality of care and resources needed.
COMMUNICATE
We are the extra eyes, ears and spokesperson for you, especially when your family member lives far away. We will provide reports, observations & consistent communication to your family members; reassuring them that you are in good hands, and everything is taken care of.
COORDINATE CARE
Once evaluations have been conducted, resources and needs identified, we will develop a personalized plan of care that meets your needs. We will coordinate services such as homecare, nursing care, therapy, home services, etc.
RELATIONSHIPS
Consider our Case Managers an extended member of your family.
We build relationships, which builds trust. If "HOME" is considered in a nursing facility, assisted living, senior community or the home you've created over the last 30 years we'll conducted wellness visits, intervene when your family member is unable to do so.
